OP14.08: Risk assessment of adnexal malignancy by benign simple descriptors and ADNEX in a two‐step strategy: a retrospective multicentre Portuguese study
Year: 2023
Type: conference-abstract
Abstract: To externally validate the IOTA two-step strategy for adnexal malignancy risk assessment by using the modified benign descriptors (MBDs), followed by the ADNEX model if the MBDs are not applicable. This retrospective analysis used data from consecutive non-pregnant ≥ 18-year-old women with at least one adnexal tumour, examined by transvaginal ultrasound and surgically managed at 3 Portuguese centres (CHULC, CHLO and HVFX) between 1 January 2016 and 31 December 2022. The tumours were classified as benign or malignant based on histology. The study endpoints were: (1) the percentage of lesions to which the MBDs were applicable; (2) the percentage of malignancy among tumours to which the MBDs were applicable; and (3) the discriminative abilities of the Two-step strategy and ADNEX (with CA125 and 10% malignancy risk cut-off, applied to all versus only tumours to which MBDs are not applicable). The study included 514 patients, evaluated by level-II and -III IOTA-certified sonologist. Histologically, 407 (79.2%) tumours were benign and 107 (20.8%) malignant. The MBDs were applicable in 26.7% (137/514 tumours, none of which were malignant). The performance of the two-step strategy and ADNEX is presented in table 1. In our patient series, the MBDs served as a reliable tool for the identification of benign lesions without computer support. The two-step strategy and the ADNEX model used in all tumours performed similarly. The two-step strategy appears to be suitable for clinical use, which should be further evaluated in prospective multicentre studies, including sonologists of all levels and differentiating pre- and postmenopausal patients.
