OP08.02: Rates of pre‐eclampsia and gestational hypertension in placenta accreta versus gestational age and BMI matched controls: a case control study

Year: 2023
Type: conference-abstract
Abstract: Placenta accreta spectrum (PAS) is characterised by deep trophoblastic implantation and extensive neovascularisation, whereas hypertensive disorders of pregnancy (HDP) are characterised by inadequate trophoblastic invasion and vascular remodelling. The purpose of this study is to evaluate the rate of gestational hypertension (GH) and pre-eclampsia (PE) in PAS vs controls. Case-control study including 155 women diagnosed with PAS by antenatal imaging or pathology, and 154 controls without PAS or placenta previa matched by body mass index (BMI) and gestational age (GA) at delivery. Differences in rates of GH and PE were compared between the two groups. Patients with PAS were older, more frequently Hispanic, and had a higher number of previous pregnancies than controls. There was a significant higher prevalence of patients with BMI ≥25 kg/m (93%, 144/155) and BMI ≥30 kg/m (62%, 96/155) among cases and controls. No differences in the prevalence of chronic hypertension and diabetes were observed. PAS cases had a significantly lower rate of GH (0.6% (1/155) vs. 7.2% (11/154); p < 0.003) and PE (1.9% (3/155) vs. 22% (34/154), p < 0.0001) compared to controls. Despite having similar risk factors, the controls with no PAS had higher rate of GH & PE. Patients with placenta accreta spectrum have lower rates of GH and PE compared to well matched controls.
