Beyond the rhythm: atrial fibrillation, diastolic dysfunction, and heart failure

Abstract: Conceptualization of diastolic dysfunction progression, AF onset, and HFpEF development Shared CV risk factors and systemic inflammation promote left ventricular and left atrial structural and functional impairment, progression of diastolic dysfunction, and AF onset. This process may eventually lead to overt HFpEF. Routine echocardiographic assessment of diastolic dysfunction is needed to identify AF patients early at a pre-clinical HFpEF stage. Early treatment strategies should target HF progression along with rhythm/rate control.
