Abstract: This chapter includes answers to practice-based questions with case studies covering the principles of diagnosis, classification, treatment, and outcomes in T-cell acute lymphoblastic leukemia/lymphoma (T-ALL/BL). T-ALL is an aggressive precursor lymphoid neoplasm, driven by malignant transformation and expansion of T-cell progenitors. The diagnosis of T-ALL versus T-LBL is arbitrarily distinguished by the presence of 25% blasts in the bone marrow or peripheral blood. Current treatment of T-ALL/BL consists of a high-intensity combination chemotherapy in suitable candidates. Early thymocyte progenitor (ETP)-ALL has been associated with a poorer prognosis in both children and adults, but with newer risk adapted approaches, the outcomes for patients with ETP-ALL is similar to those with classical T-ALL/LBL. Immunophenotype subtypes of T-ALL reflect the different stages of differentiation of normal thymocytes based mainly on membrane expression of various antigens. Although the majority of adult ALL patients reach complete remission, many will eventually relapse and subsequently be much less responsive to salvage therapy.
