Abstract: This article examines the transformations that followed the inspection of Governor General N. N. Muravyov in the entrusted territories. Against the backdrop of the global task of restructuring the administrative-territorial system, changes have occurred in the management of social issues, in particular, medical affairs. Thus, the simplified management system of the Yakut, Kamchatka, Transbaikal regions, providing for the independence of governors, contributed to the introduction of the positions of medical inspectors and the formation of a staff of medical officials. The most significant changes took place in the medical sphere of the Trans-Baikal region, where medical care for the military contingent began to form.
