Abstract: We thank Garcia and Spratt 1 for providing their insights regarding our surrogate analysis focusing on two intermediate clinical end points (ICEs) of overall survival (OS): radiographic progression-free survival and clinical PFS, which have the potential to expedite the evaluations of new strategies to treat men with metastatic hormone-sensitive prostate cancer.2 The authors raise the important concern of inadvertent adoption of an ICE predominantly identified within a therapeutic context that no longer reflects the current landscape.Furthermore, they also expertly advise the need to ensure putatively ICE(s) effectively capture(s) surrogacy in terms of OS, and account for potential differences in drug classes and the impact of subsequent therapeutic options. In conclusion, we concur with the authors' cautious approach before adopting ICEs in phase III trials, emphasizing the need for robust validation in contemporary patient populations.
