Abstract: Complete heart block (CHB) requiring permanent pacemaker (PPM) affects 7.4% to 13.5% of children undergoing mitral valve replacement (MVR) and is associated with increased mortality.1-3 Pediatric MVR is especially high risk because it is typically reoperative, annular upsizing may be necessary, and conduction location varies with underlying anatomy. We have adopted intraoperative conduction mapping (IOM) to identify and avoid conduction pathways during complex congenital repairs.4 Here we review the feasibility and utility of IOM during MVR.
