E-235 Outcomes of carotid covered stents in cancer patients: a retrospective analysis

Year: 2024
Type: conference-abstract
Abstract: Purpose To evaluate the safety and efficacy of covered stents placed within the carotid artery. Materials and Methods A single-institution retrospective analysis of patients who underwent covered stenting placement with GORE Viabhan covered stent from September 2016 to February 2024 was performed. Results 8 patients who had 10 procedures for a total of 14 covered stents were included in this study, with a median age of 66(range: 47–71). The median follow-up time was 10 months. The majority of primary cancer diagnoses were SCC(6/8), with the other two being papillary thyroid carcinoma(1), and one patient who had CML and Takayasu arteritis. All patients had a history of prior chemotherapy (100%), and the majority had radiation and surgery to the head and neck (90%). Indications for placement of the carotid stenting were for blowout (7/10,70%), pseudoaneurysm (2/10,20%), and symptomatic carotid stenosis (1,10%), of which when carotid balloon test occlusion was performed, 78% had failed their occlusion test. A total of 14 stents were placed, of which the median diameter was 8 mm(range:7–9 mm). The median Kaplan-Meier overall survival was 172 days. The median complication-free survival per Kaplan-Meier was 153 days. Overall complications of the stents included stent infection (1,10%), rupture (1,10%), and thrombosis (4,40%). The remaining stents remained patent, but these patients died within 6 months. Univariable logistic regression demonstrated no predictors of complication rates. Conclusion Carotid covered stents are placed for high-risk patients and may have an increased risk of complications in cancer patients in an acute setting. Disclosures K. Pal: None. C. Young: None.
