Abstract: Case 2: A 28-year-old female with relapsing lupus nephritis A young female, 28 years, received a diagnosis of systemic lupus erythematosus at the age of 11 years (in London, UK), it was characterized by the presence of rash, arthritis, fever, and lymphadenopathy, with typical anti-dsDNA antibodies and low complement levels. She had several flares in 2006, 2007, 2009 (skin, serositis, vasculitis digits) and was treated with hydroxychloroquine, azathioprine and steroids, including methylprednisone several times. In 2011 she was diagnosed with lupus nephritis (LN) (WHO Class IV) and was treated with methylprednisone and mycophenolate mofetil (MMF) with subsequent complete remission of LN.In 2014 she had a first relapse of LN, and was treated with rituximab and methylprednisone, and stopped MMF.In 2016, after moving to The Netherlands, she had a second relapse of LN, initially treated with EURO lupus scheme (low dose cyclophosphamide) and thereafter with MMF.In 2019, she had a third relapse, and was treated with steroids, MMF and tacrolimus (‘triple therapy’). She stopped tacrolimus in 2021 because of complete remission. Her current serum creatinine is 50 µmol/L with an EGFR >90 mL/min/1.73m2 and she has a urinary protein/creatinine ratio of 23 mg/µmol creatinine.Discussion Points What would be the treatment options for retaining remission of LN?What would be the treatment options if a 4th relapse occured?Learning Objectives At the end of this workshop participants will be able to:• Discuss the use of anti-B cell therapy and other potentially treatments in the future for refractory/relapsing LN
