EP06.67: Ultrasonographic diagnosis of fetal single umbilical artery with malformation at 11–13+6 weeks of pregnancy and analysis of pregnancy outcome
Year: 2024
Type: conference-abstract
Abstract: To investigate the diagnostic value of prenatal ultrasound in fetal single umbilical artery (SUA) at 11-13 weeks gestation, and to analyse the correlation between SUA and fetal structural abnormalities and chromosomal abnormalities. From July 1, 2019 to June 30, 2022, the clinical data of 181 fetuses who were diagnosed as single umbilical artery by ultrasound during early pregnancy in Fujian Maternity and Child Health Hospital were retrospectively analysed, To analyse their comorbid structural malformations and chromosomal abnormalities and to follow up their pregnancy outcomes, and to investigate the value of ultrasound diagnosis of SUA in early pregnancy, and the causes of missed diagnosis and misdiagnosis during early pregnancy were analysed. Among 181 SUA fetuses, isolated single umbilical artery (iSUA) was 51.93% (94/181) and non-isolated single umbilical artery (niSUA) was 48.07% (87/181). In niSUA, 56.32% (49/87) with other soft index abnormalities and structural malformations, 35.63% (31/87) combined with other soft index abnormalities alone, and 8.05% (7/87) with simple merge structure exception. Chromosomes abnormal rates of iSUA and niSUA were 9.43% (5/53) and 25.53% (12/47) respectively. The live birth rate was 90.43% in iSUA group and 34.48% in niSUA group. There was significant difference in live birth rate between the two groups(c=61.034, P < 0.001). The specificity, sensitivity, positive predictive value and negative predictive value were 99.96%, 75.14%, 90.67% and 99.86% respectively. Ultrasonography in early pregnancy is of great value in the diagnosis of fetal SUA. SUA was associated with fetal structural abnormalities and chromosomal abnormalities. The incidence of chromosomal abnormalities in iSUA was lower than that in niSUA. The prognosis of SUA alone is better than that of severe structural abnormalities or chromosomal abnormalities.
