Abstract: Endometrial cancer is one of the most common cancers among women. A case of a 23-year-old nulligravid with irregular menstrual bleeding. She was initially managed conservatively. Ultrasound revealed thickened endometrium 10.3mm, non-uniform, heterogeneous, with irregular cystic areas, indistinct midline echo, irregular endo-myometrial junction at the anterofundal area, colour score 3, moderate flow on power Doppler. The sonographic impression then was a normal sized retroverted uterus, thickened heterogeneous endometrium with probable myometrial invasion of more than 50%, diffuse adenomyosis, serous cyst, right ovary, benign by IOTA easy descriptors, normal left ovary. CT scan revealed no discrete enlarged lymph nodes. Unremarkable adjacent structures with linear densities in the posterior basal segment left lung. Patient underwent robotic assisted laparoscopic hysterectomy, right salpingo-oophorectomy, with ovarian tissue cryopreservation, left salpingectomy and bilateral lymph node dissection. No ascites. Uterus measured 6x 7x 5 cm, endometrium measured 1.0mm, with no residual masses. Right ovarian tissue sent for cryopreservation. Histopathology revealed residual endometrial endometrioid carcinoma, not otherwise specified, with squamous adenosinophilic metaplasia, FIGO grade 1, in a background of endometrial hyperplasia. Tumour confined to the endometrium with residual carcinoma size of 1 centimetre in single widest dimension. Uterine serosa, lower uterine segment, cervical stroma, and lymphovascular invasion not identified. The right and left fallopian tube and right and left parametria negative of tumour. The right and left pelvic lymph nodes were negative of tumour. Please note: The publisher is not responsible for the content or functionality of any supporting information supplied by the authors.
