EP07.34: Prognosis and outcome of prenatally diagnosed fetal ovarian cysts

Year: 2024
Type: conference-abstract
Abstract: Ovarian cysts are the most common cystic abdominal mass in female fetuses and neonates. Its incidence is considered to be 1 in 3000 female newborns. The outcome of ovarian cysts is variable. Torsion, complications secondary to hemorrhage or mass effect are some of its possible manifestations. However, most simple ovarian cysts usually resolve spontaneously. Here we describe the perinatal outcomes of newborns with prenatal diagnosis of ovarian cyst in the last 10 years at the Hospital Clínico Universitario de Salamanca. This is a retrospective study through the review of medical records of newborns prenatally diagnosed with fetal ovarian cysts in the last 10 years at the Hospital Clínico de Salamanca. To know the evolution of the newborn, telephone contact was made with the family and the clinical history of the child was reviewed in those cases in which it was available. During the last 10 years, 12 cases of fetal ovarian cysts have been reported in our centre. 11 were isolated findings and all were less than 5 cm in size. Only 1 case, with a cyst measuring 9x8 cm, was associated with hydramnios. 42% solved spontaneously, another 42% remained unchanged by ultrasound until birth and 26% increased in size, requiring immediate postnatal surgery for severe abdominal pain. In the cases of cyst persistence, the diagnosis of ovarian or adnexal cyst was confirmed in all but one of them, which turned out to be a mesenteric cyst. None of the cases was associated with genetic conditions. 75% of the girls were born at term by vaginal delivery versus 25% who were born by Caesarean section. The reasons for Caesarean section were not related to the presence of the cyst. Ovarian cysts are rare findings in prenatal life In our case series, the rate of complication and postnatal resolution has been similar to that reported in the literature, and is mainly related to the size of the cyst at birth. Prenatal torsion of the cyst is a rare complication, more common if it is larger than 5cm.
