Abstract: Identification of ultrasound signs prognostic of postnatal surgery in fetal ovarian cysts. Ultrasound assessment of 122 fetal ovarian cysts and analysing of their outcomes; method of multiple logistic regression. The following ultrasound sings of fetal ovarian cysts can be considered prognostic regarding postnatal torsion: the upper pole of the cyst is up to the lower edge of the fetal liver (β = 0.463); the thick capsule of the lesion (β = 0.301), the presence of hyperechoic components (β = 0.21), size of the lesion more than 4 cm (β = 0.174) (F = 7.80; df = 8.11, p < 0.05). The regression equation for postnatal ovarian loss is as follows: Y = 0.004110+ 0.371505*the upper pole of the cyst is up to fetal liver+0.298958*hyperechoic component+0.292902*thick capsule. Some ultrasound features were identified as being predictive of surgical treatment (not only because of torsion) in newborns in cases of prenatally diagnosed ovarian cysts: the upper pole of the lesion is up to the lower edge of the fetal liver (β = 0.397); visualisation of the thick capsule (β = 0.308); cysts size is more than 4 cm (β = 0.143) (F = 19.93; df = 8.11, p < 0.05). So, when the fetal ovarian cyst is diagnosed, the risk of surgical treatment after birth can be determined using the regression equation: Y = 0.0727 + 0.43 * upper pole of the lesion reaches the edge of the fetal liver + 0.4047 * thick capsule of the cyst. Ultrasound evaluation helps to predict torsion and postnatal surgical intervention in case of prenatally diagnosed ovarian cysts, may help to analyse the need of fetal surgery and will provide more comprehensive prenatal counselling.
