EP12.12: Agreement between two different devices for maternal hemodynamic evaluation

Year: 2024
Type: conference-abstract
Abstract: This study aimed to assess the agreement between maternal hemodynamic measurements taken with two different devices for maternal hemodynamic evaluation. Overall, 344 measurements from 172 patients (156 healthy controls, 11 patients with small for gestational age fetuses and 5 with fetal growth restriction) enrolled at the Imperial College, London were considered. The agreement between USCOM® and Vicorder® measurements was evaluated for Heart Rate (HR), Cardiac Output (CO), Stroke Volume (SV) and Total Peripheral Resistance (TPR). The assessment was based on Bland-Altman plots with 95% limits of agreement (LOA) for the mean difference (MD) between two devices, reporting also the intraclass correlation coefficient (ICC). In general, measures from different devices were poorly correlated, except for HR (ICC = 0.76, p < 0.001). In particular, ICC was 0.21 for TPR, 0.28 for SV and 0.29 for CO. Wide LOA were found for all measurements. USCOM® provided, on average, higher values for HR (MD = -0.30), CO (MD = -0.47) and SV (MD = -5.13) and lower for TPR (MD = 72.7), compared to Vicorder®. The agreement between USCOM® and Vicorder® measurements is low, emphasising the high risk of bias in using measurements from different devices without standardisation. Thus, when data from different devices are compared, standardisation based on reference ranges should be considered (i.e. z-scores, modified z-scores, MoMs or similar).
