Abstract: Various cutaneous and subcutaneous nodules can be detected in the fetus by prenatal ultrasound examination. Most of these are benign and self-limiting tumours but may affect prenatal and postnatal management. Infantile fibrosarcoma (IFS) is a very rare tumour that usually occurs in fetuses-infants, characterised by high local aggressiveness but overall high survival. We presented the prenatal findings and postnatal outcomes of a prenatally detected IFS case. A 37-year-old pregnant woman with gravida 4 parity 3 was referred due to a fetal cranial tumour at the 36th gestational week. Sonographic evaluation revealed a well-circumscribed, hyperechoic, homogeneous mass with dimensions of 59x76mm extending from the right frontolateral part of the fetal cranium. The tumour had no association with intracranial structures. No additional anomaly was detected in the fetus. Magnetic Resonance Imaging examination also showed a tumour originating from the cranium without intracranial extension. A 2,600g male newborn was delivered by Caesarean section at the 37th gestational week. The newborn was followed up in the neonatal intensive care unit and operated on the 10th day. No complications occurred after the operation. Pathological evaluation of the tumour was interpreted in favour of IFS, one of the subtypes of malignant mesenchymal tumours. Although locally infiltrative, IFS rarely metastasises and generally has a good prognosis. Although IFS is defined especially in the infantile period, its inclusion in the differential diagnosis of fetal tumours in the prenatal period is important in predicting the prognosis of these cases.
