Abstract: Sirenomelia or “mermaid syndrome” is a very rare and usually lethal sequence of congenital anomalies with incidence of one in 100,000 pregnancies. This case study presents a 25 yr old female came to radiology department for routine anomaly scan at 19 weeks of gestation. A case of sirenomelia was identified, with prenatal ultrasound being the primary modality for diagnosis. Associated anomalies included multicystic dysplastic kidney, sacrococcygeal agenesis, lateral abdominal wall defect and anal atresia. Prognostic factors such as gestational age at diagnosis, and presence of associated anomalies significantly influenced survival rates. Despite advancements in medical care, the overall prognosis for infants with sirenomelia remained guarded, with high mortality rates observed. Sirenomelia remains a challenging congenital anomaly with significant implications for diagnosis, management, and prognosis. Prenatal imaging modalities play a crucial role in early detection, allowing for timely intervention and improved outcomes.
