EP08.25: Cerebral‐placental‐uterine ratio at early third trimester to predict risk of fetal growth restriction

Year: 2024
Type: conference-abstract
Abstract: To evaluate the performance of cerebral-placental-uterine ratio (CPUR) for prediction the risk of FGR, as earlier detection of the problem might lead to better surveillance and improvement in perinatal outcomes. A prospective cohort study was conducted in pregnant women at GA 28-31 wk with EFW at 10 – 90 centile between May and December 2023. Doppler studies were measured to calculate for CPR and CPUR. After delivery the newborns were grouped into AGA and SGA groups. CPUR as well as other parameters were compared between the two groups. A total of 228 pregnant women, 51 (22.7%) had SGA at birth. SGA newborns were more likely to have lower EFW centile, lower AC centile, lower CPR centile, and lower CPUR. The AUC of ROC curve was 0.59 and the optimal cut-point of CPUR using Youden's J statistic was 2.1, with sensitivity of 88.7% and sensitivity of 37.4%. Stepwise logistic regression analysis showed CPUR<2.1 was an independent predictor of SGA with OR 3.4 (95% CI 1.5-7.7). CPUR at early third trimester is an independent predictor associated with SGA fetus, with high sensitivity for early detection to prevent adverse consequences.
