F087 Sarcopenia risk in Huntington’s disease

Year: 2024
Type: conference-paper
Study design: Observational Study
Abstract: Background Sarcopenia is defined by low levels of muscle strength, muscle quantity/quality, and, physical performance (1). Although sarcopenia in clinical cases is observed in Huntington’s disease (HD) the clinical diagnosis is still challenging. The Consensus 20191 recommends 2 tools for early identification of cases at risk: Strength, assistance walking, rise from a chair, climb stairs, and fall questionnaire (SARC-F) (2) and the Ishii Test (3). Aims To assess the concordance between SARC-F and SARC-CalF (4) vs. Ishii test in HD. Methods/Techniques Cross-sectional and observational study. SARC-F is a 5-item questionnaire that is self-reported by patients based on the self-perception. SARC-CalF uses the same scores as SARC-F plus calf circumference measurement. On the other hand, Ishii test included age, grip strength, and calf circumference, assessed by health personnel. To analyze concordance, we used the Cohen´s kappa coefficient (CK Coef.). Results/Outcome Thirty-seven self-ambulatory participants, 60% women, CAG 43 ± 2.76, age 53.4 ± 11.9 years, BMI 24.3 ± 2.9, 62% moderate-high adherence Mediterranean diet, UHDRS motor 29.8 ± 22.1 and TFC 9.6 ± 3.2. The CK coef. were SARC-F vs. Ishii test 0.36 (p<0.05), and SARC-CalF vs. Ishii test 0.55 (p<0.05). Conclusions In HD, compared to the Ichii test, SARC-F cannot identify early cases at risk for sarcopenia. However, SARC-CalF had higher concordance with the Ichii test compared to SARC-F. The identification of cases at risk of sarcopenia is important and needs early lifestyle interventions, including physical activity and Mediterranean diet. Further research is needed to confirm these findings and the benefits of early lifestyle interventions.
