Abstract: English summary Borderline composite restorations Nor Tannlegeforen Tid. 2024; 134: 708-11. Treatment choices for restoration of extensive dental defects can be challenging, and may be influenced by a number of factors. In recent years, a change in treatment strategy has been seen, where new techniques and better materials have enabled a more minimally invasive way of thinking. Case study: The patient presented with a lost amalgam restoration in a symptom-free endodontically treated tooth 16. The root fillings were incomplete, but without apical pathology. It was decided to restore the tooth using direct composite. Discolored tooth substance was prepared, and the palatal wall reduced by 3 mm. Temporary composite buds were made to facilitate rubber dam placement. The restoration was built up by using sectional matrices, a 3-step etch-and-rinse system, and a microhybrid composite. Conclusion: Uncertain prognosis, economy and preservation of tooth substance are factors that favor the choice of direct restoration with composite, while technical challenges such as moisture control and anatomical shaping of the restoration can be disadvantages. Clinical studies show that cusp-covering resin-based composite restorations can be a long-lasting alternative to crown therapy.
