Abstract: Disclosure: R. Tarrazona - Dominisac: None. M. Lantican: None. J.B. Aragon: None. Overt Cushing's syndrome and primary aldosteronism are two uncommon yet distinct adrenal disorders. Rarely do they coexist, complicating diagnosis and management. Here, we present a case of a 42 year old woman with a complex clinical profile for the past 21 months, marked by amenorrhea, weight gain, abdominal enlargement, new-onset hypertension, hyperglycemia, proximal myopathy, easy fatigability, and difficulty concentrating. A comprehensive physical examination and endocrine work-up confirmed coexistence of Cushing’s syndrome and primary hyperaldosteronism due to a left adrenal adenoma. Laparoscopic unilateral adrenalectomy was performed as definitive treatment of the functional tumor which resulted in substantial clinical improvement thereafter. This case highlights the importance of precise diagnosis and appropriate management in rare combined disorders, ultimately ensuring favorable patient outcomes. Presentation: 6/1/2024
