838 Outcomes of a pilot educational program for frontline clinical staff in early detection and intervention in immune-related adverse events

Abstract: Background The increased use of immune checkpoint therapies (ICTs) has led to augmented rates of immune-related adverse events (irAEs), which often require inpatient hospitalization. There is a need for educational programs directed to frontline clinical staff, who are the initial point of contact with patients to detect the early signs and symptoms of irAEs to facilitate prompt intervention. Herein, we report the development of a pilot irAE educational program targeted at clinical nursing staff in the Department of Genitourinary Medical Oncology at the University of Texas MD Anderson Cancer Center. The objective of this program is to improve early detection and intervention in irAEs. Methods A comprehensive curriculum (figure 1) has been developed covering the mechanism of action of ICTs, pathophysiology, and management of specific irAEs (including common irAEs such as colitis and rare irAEs such as myocarditis), and complications of immunosuppressants. Teaching is provided monthly by advanced practice providers (nurse practitioners and physician assistants) and clinical pharmacists. The primary endpoints of this study are: 1) knowledge assessments through pre- and post-testing; 2) rates of preventable hospitalizations attributable to irAEs. A standardized evaluation form will obtain feedback from learners on satisfaction, perceived usefulness, and material relevance. Attendance at the presentations will also be evaluated to determine which clinic teams are present and whether the program is targeting the frontline clinical staff. Results The pilot program has been launched and preliminary effectiveness data are being collected on an ongoing basis. If successful, this innovative peer-to-peer education program will improve clinical outcomes through reduction of irAE hospitalizations and provide a model for use at other cancer centers.
