Abstract: Hashimoto’s encephalopathy is an autoimmune disorder characterized by elevated thyroid autoantibodies in serum or cerebrospinal fluid, accompanied by a variety of neurological symptoms including changes in consciousness, cognitive impairment, visual hallucinations, seizures, and stroke-like symptoms. The responsiveness to steroid therapy makes it a critical differential diagnosis to consider. We present a rare case of a 60-year-old woman who initially exhibited myoclonus, later developed cognitive decline, and showed positive response to steroid treatment. Despite initial improvement, her symptoms of myoclonus returned 3 months after discontinuing steroids, leading to a restart of the treatment. The renewed steroid therapy improved her condition once again.
