Thoracic branched endoprosthesis landing in zone 1 with left carotid-subclavian bypass and left common carotid artery ligation

Type: article
Abstract: Thoracic endovascular aortic repair (TEVAR) has gained popularity in the treatment of descending aorta aneurysms and dissections because of its minimally invasive approach and effectiveness.1,2 However, conventional TEVAR requires modifications with the involvement of aortic arch vessels.3,4 Using a thoracic branched endoprosthesis (TBE; W.L. Gore & Associates) facilitates endovascular repair of aneurysmal disease in the aortic arch.4 For adequate zone 2 landing, the instructions for use for a TBE require a proximal segment length, the distance between left common carotid artery (LCCA) and the left subclavian artery (LSA), to be at least 2.5 cm to avoid compromising the proximal seal.
