Abstract: Introduction Cardiac myxoma is a rare tumour of the heart occurring in the left atrium (75% of cases). In this short case study, a mass in the right atrium is discussed. The MRI techniques and outcomes are explained. Materials and Methods A 82 year old attended for a CMR to help characterise a lesion seen on echocardiography. An anatomical stack and routine cine SSFP imaging were performed on a Siemens 1.5T Avanto Fit. Then T1 mapping, perfusion, early and late gadolinium for better visualisation of the lesion along with the rest of the heart. Results Imaging findings were of a mass arising from the inferior wall of right atrium, measuring approximately 20 × 13 mm. The mass did not perfuse. On late gadolinium imaging the mass showed ring like enhancement. The SMOLLI T1 mapping displayed signal analogous to myocardium. The mass characteristics were most in keeping with a myxoma. There was moderate aortic stenosis, normal biventricular function with no fibrosis. Discussion The cine sequences help locate the mass and visualise the surrounding anatomy. The mapping technique characterised the mass and the perfusion helped display the vascularity of the lesion. It was also noted the patient had moderate aortic stenosis. Conclusion The use of all imaging planes and sequences proved valuable to help locate and characterise the heart mass. This case underlined the role CMR has in imaging of masses.
