Abstract: Mirzaeva Adiba Damirovna Head of the Department of Pediatrics Republican Scientific Center for Emergency Medical Care 抽象的。 Abstract. The data of long-term continuous monitoring of the oxygen saturation index in 36 school-age patients (7-18 years old) with severe concomitant traumatic brain injury (SCTBI) admitted to the intensive care unit (ICU) of the neurosurgical department of the Republican Scientific Center for Emergency Medical Care (RSCEMC) were studied in the first hours after a traffic accident (RTA) -33, catatrauma -3 patients. From the first hours of intensive care for severe combined traumatic brain injury in children older than seven years, the oxygen saturation index could be maintained at a level above 97.8%,regardless of the severity of the injury. The average hourly rates of oxygen saturation in the circadian rhythm were the highest in patients of group 3, which is associated with more active insufflation of concentrated oxygen-air mixtures due to the severity of the condition.A strong direct correlation was found between oxygen saturation Scientific research of the SCO countries: synergy and integrationInternational Conference and SBP (0.7), SV (0.66) and PBP (0.79) in group 2, which confirms the tendency to improve blood oxygenation with an increase in SV, SBP, PBP with a tendency to decrease in heart rate. Keywords: severe concomitant traumatic brain injury, circadian rhythm, oxygen saturation, school-age children.Relevance.The growth of traumatism increases not only the frequency, but also the severity of traumatic brain injury (TBI), which in 50-70% is combined with extracranial injuries.Mortality from concomitant traumatic brain injury (CTBI) ranges from 12 to 69%.The clinical picture and prognosis in acute combined TBI are determined by the localization, severity and volume of cerebral and extracranial traumatic injuries. The treatment of severe TSBI is based on careful and comprehensive intensive care, which includes a multimodal, protocolized approach that includes meticulous hemodynamic support, respiratory care, fluid management, and other aspects of therapy aimed at preventing secondary brain damage. The main goals of intensive care are prevention and treatment of intracranial hypertension and secondary brain injury, maintenance of cerebral perfusion pressure, and optimization of cerebral oxygenation. It has been established that an increase in the depth and frequency of external respiration in TSBI as a mechanism for compensating for developing hypoxia; an increase in cardiac output, intensification of tissue metabolism in order to mobilize additional energy resources can lead to secondary damage to the injured brain, the development of multiple organ failure syndrome [1][2][3][4][5].Due to the lack of information on changes in the oxygen saturation index, data on the reasons for the insufficient effectiveness of cerebral circulation correction, we made an attempt to study the features of changes in the oxygen saturation circadian rhythm index in the acute period of SCTBI in children aged 7.1-18 years. Purpose of work.To study the circadian rhythm of the oxygen saturation index in severe concomitant traumatic brain injury in children older than seven years.Material and research methods. The parameters of a comprehensive examination of 36 patients of school age (7-18 years old) with severe concomitant traumatic brain injuries (SCTBI) admitted to the intensive care unit (ICU) of the neurosurgical department of the Republican Scientific Center for Emergency Medical Care (RSCEMC) in the first hours after road traffic accident (RTA) -33, catatrauma -3 patients.Continuous hourly monitoring of hemodynamic parameters, including SBP, was carried out for 30 days after STBI.According to the indications, patients were started on invasive mechanical respiratory support (MRS) at admission.Mechanical respiratory support was started on artificial lung ventilation (ALV) for a long time, with subsequent transfer to SIMV.Restoration Scientific research of the SCO countries: synergy and integration International Conference 18 98,8±0,2 19 98,6±0,2 20 98,4±0,2 21 98,4±0,2 22 98,4±0,1 23 98,1±0,2* 24 98,2±0,3 25 98,5±0,3 26 98,3±0,2 27 98,6±0,4 28 98,3±0,3 29 98,3±0,2 30 98,6±0,3 *-certainly relatively score on the first day ‴-significantly relative to indicator in group 1 Table 2 Average Hourly Circadian Oxygen Saturation Hours 1 group 2 group 3 group Oxyg.sat/BH 0,06 -0,03 -0,34 Oxyg.sat/HR 0,59 -0,56 -0,14 Oxyg.sat/OVT 0,55 0,26 0,03 Oxyg.sat/SAD 0,48 0,70 0,20 Oxyg.sat/DAD 0,24 -0,18 -0,29 Oxyg.sat/PP 0,57 0,79 0,43 Oxyg.sat/MVP 0,60 0,10 -0,17 Oxyg.sat/PR -0,05 -0,18 -0,34 Oxyg.sat/IRT 0,38 0,48 0,20 Oxyg.sat/SV -0,10 0,66 0,55 Oxyg.sat/MAP 0,45 0,46 0,04 Oxyg.sat/TS 0,76 0,85 0,59 Scientific research of the SCO countries: synergy and integration
