Abstract: The Achilles heel of revascularization in chronic limb-threatening ischemia is that a balloon is sometimes unable to cross the severely calcified below-the-knee lesion. We presented a new technique for crossing this lesion using the blunt needle endoluminal cracking over the strained through and through wire (BECOST) technique.
