Abstract: A 62-year-old man with acute coronary syndrome underwent an echocardiogram for evaluation. A transthoracic echocardiogram revealed masses within a giant coronary sinus. This case highlights the role of multimodality imaging in investigating coronary sinus enlargement, coronary sinus thrombosis, and the approach to management.
