Abstract: The South Asian genetics alongside the proposensity to female obesity and Type 2 diabetes mellitus, and the rising incidence of advanced maternal age at conception, have contributed to Hyperglycaemia in Pregnancy (HIP) evolving into a common medical encounter for physicians and obstetricians.1 The diagnosis and management of HIP is unique to pregnancy, where normoglycaemia from pre-conception and throughout the pregnancy as per the recommended blood sugar targets ensures significant reduction of maternal and fetal adverse clinical outcomes, that is similar to a non-diabetic pregnancy outcome.A planned pregnancy with timely discontinuation of medications that are potentially teratogenic (other than metformin) is crucial, while achieving the recommended blood sugar targets is challenging. We provide guidance for the Sri Lankan physicians managing HIP, based on current scientific evidence and globally accepted management guidelines.
