Abstract: Background: There are few recent studies on the status of therapeutic drug monitoring (TDM) and vancomycin pharmacokinetic consultation service (PKCS) in Korea. We surveyed the current TDM status and details of vancomycin PKCS in clinical laboratories. Methods: In August 2023, a questionnaire survey was sent via email to 170 clinical pathologists (CPs) (one per institution). The number of questions varied depending upon the response of the CPs.The results of the survey were then analyzed. Results: Responses were obtained from the CPs of 54 institutions. The proportion of institutions measuring drug concentrations in-house without referral was high for vancomycin (74.1%), valproic acid (63.0%), tacrolimus (61.1%) and cyclosporine (46.3%), but low for amikacin (11.1%) and gentamicin (7.4%).In the past year, the drug most commonly requested for PKCS (N of institutions) was vancomycin (37), followed by valproic acid (18), digoxin (17), amikacin (14), and gentamicin (11).For dosing simulation, Abbottbase PKS and MwPharm++ were commonly used (N=14 each).Out of 27 institutions that provided vancomycin PKCS by the CPs, 18 institutions implemented area under the curve (AUC) estimate as a target index for vancomycin. The common challenges for adopting AUC in clinical practice included a lack of personnel providing PKCS and difficulty selecting or using software programs. Conclusions: CPs provided a significant portion of vancomycin PKCS with AUC-based vancomycin dosing. This study provides fundamental data for establishing standardized TDM guidelines.
