Abstract: Carotid cochlear dehiscence (CCD) is a rare phenomenon that occurs due to abnormal thinning of the bony plate separating the internal carotid artery and the cochlea within the petrous segment of the temporal bone. In this report, we describe a unique case of bilateral carotid cochlear dehiscence due to lateralization of the internal carotid artery. We examine the variable presentation of CCD and review the factors that contributed to the development of a bilateral carotid cochlear dehiscence. Identification of CCD as a rare pathology remains imperative since it often mimics other inner ear pathologies and exhibits significant risk in surgical intervention.
