Abstract: Background: Though HLA matching increases graft survival in deceased donor kidney transplant (DDKT) recipients, it has been found to amplify racial disparities due to differences in the HLA make up of donor and candidate populations. Thus, HLA matching priority has been reduced several times in the allocation system. We developed a novel metric of personalized immune compatibility likelihood (PICL) that measures each candidate’s likelihood of having donors in various antigen level risk categories and propose allocation priority adjustment based on PICL to increase the rate of well-matched DDKT without exacerbating racial disparities.
