Abstract: Immunogenicity, or a substance’s potential to elicit an immunological response, is an important component in the creation of biologic and small molecule medications. Immunogenicity can impair the efficacy of biologics such as monoclonal antibodies, therapeutic proteins, and gene treatments, as well as pose possible safety issues and cause therapeutic failure. Understanding and predicting immunogenicity is thus a critical component of current pharmaceutical development. This study investigates the importance of immunogenicity in drug development, the variables that influence immunogenic reactions, methodologies for measuring and managing these responses, and regulatory implications. In addition, we cover recent advances in immunogenicity prediction and monitoring, as well as the issues associated with immunogenicity in emerging medication classes. The review looks at real-world examples that show how immunogenicity may significantly affect pharmacokinetics and pharmacodynamics. Additionally discussed are the regulatory standards governing the assessment of immunogenicity and the necessity of in corporating this data into regulatory filings. Lastly, future studies on immunogenicity will have an impact on the therapeutic impact landscape, which will have consequences for patient-centered care and precision medicine.
