Abstract: IntroductionThe incidence of malignant transformation is 1.5% to 2% in mature cystic teratoma (MCT). Neuroectodermal tumors in the ovary are rare cases, of which only a few have been reported so far. In this report, we present a case of an extremely rare neuroectodermal tumor, ganglioglioma (GG), which occurred in an MCT and only two cases have been reported before.Case ReportThe patient, a 30-year-old woman with no prior medical history of brain tumors or neurological disorders, was found to have a cystic and solid mass within a right adnexal cyst, accompanied by the formation of mural nodules as revealed by magnetic resonance imaging (MRI). The histopathological examination revealed typic pathological features of mature cystic teratoma like mature squamous epithelium and cutaneous appendages. A mural nodule in teratoma exhibit an intricate proliferation and intermingling of glial cells and neurons, with notable cellular atypia. Notably, neuronal cells exhibited binucleation and focal distribution, with the occasional presence of Nissl bodies. Immunohistochemical analysis corroborated the dual components of the tumor, GFAP was positive in glial cells, chromogranin A (CgA), CD34, Neu-N, and Calretinin (CR) were positive in neurons. The tumor demonstrated a low Ki67 index, indicative of a slow proliferative rate, and harbored a wild-type P53 genotype without mutations in IDH1, while BRAFV600E was negative.ConclusionsThe diagnosis of GG is a rare occurrence in MCT. The accumulation of cases is crucial for developing an effective treatment strategy for this rare and malignant transformation in teratoma.
