Abstract: Aortic valve reimplantation with the Valsalva graft is a well-established procedure for aortic root replacement with sinuses reconstruction.1 In 2014, El Arid and colleagues2 reported a modification to the standard reimplantation that avoids the secondary hemostatic suture by simply attaching the top of the commissures to the new sinotubular junction (STJ). Drawbacks are the diastolic stress concentrated at the sutures holding the commissural posts and the lack of 3 individual sinuses caused by blood flowing behind the commissures.
