Abstract: Impacted teeth, such as lower premolars, are often identified in routine exams and can cause discomfort and complications, such as infections and cysts. Literature notes these teeth may require surgical interventions for removal. The choice of surgical access technique should carefully consider the preservation of vital structures. This work aims to report a clinical case illustrating the lingual surgical approach for impacted lower premolars, emphasizing surgical planning for safe, effective removal. A 24-year-old male presented with discomfort in the right mandibular lingual area. Clinical and initial radiographic exams revealed an impacted tooth, with CT scans confirming angulation. Treatment involved careful osteotomy to protect vital structures and adjacent roots, performed under local block anesthesia with precise techniques to minimize complications. The postoperative course showed no paresthesia, and the patient experienced satisfactory recovery with appropriate healing. Technique selection must consider depth, tooth angulation, and the risk of adjacent structural damage, underscoring the need for detailed surgical planning. The lingual approach for impacted tooth extraction was successfully performed without complications or paresthesia. Careful technique and precise anatomical handling ensured patient recovery.
