Abstract: • Transient chylomicemia caused by a high-fat diet can lead to pseudoconcentrations of mitotane. • Chylomicra should be separated through freezing and centrifugation, allowing for the measurement of true concentrations of mitotane. • VLDL is a key factor influencing the distribution of mitotane, and related metabolites can guide individualized dosing regimens.
