Not All Heart Failure Improves With Diuresis: A Lesson in Left Ventricular Assist Device Physiology
Year: 2025
Type: article
Abstract: Although heart transplantation is considered the optimal therapy for end-stage heart failure, there are not enough donors to meet the rising demand. For patients with refractory heart failure who require additional hemodynamic support, left ventricular assist devices (LVADs) may be an option. Transthoracic echocardiography (TTE) remains the initial modality of choice for routine LVAD evaluation and speed optimization. On the basis of clinical trial data, 25% of patients with continuous-flow LVADs experience New York Heart Association class II to IV symptoms, likely due, in large part, to inadequate left ventricular unloading.
