Abstract: The mechanisms of acute rejection and its diagnosis are analyzed, and we concluded: The diagnosis of rejection requires a high level of suspicion, knowledge of the patient’s situ- ation and access to different diagnostic approximation techniques. Diagnosis and treatment must be early. Biopsy remains the gold standard for diagnosis. New methods for early diagnosis are in the study and in validation, with many limitations for massification of these techniques and possibly of implementation in our environment. The best prevention of rejection is the selection of a good donor, adequate immunosuppres- sion and monitoring of this, which should be of very low intensity favoring rejection, or of very high intensity exposing the patient to toxicity from it.
