Abstract: In the United States, alpha-gal syndrome (AGS) is primarily associated with lone star tick (Amblyomma americanum) bites. We describe AGS onset after an Ixodes scapularis tick bite and present AGS surveillance in Maine, 2014-2023. US health and public health professionals should be aware of AGS outside the established lone star tick range.
