Abstract: Introduction Gold Standards Framework (GSF) supports clinicians in identifying patients in their last year of life to aid in the development of proactive and personalised plans for care including Advance Care Plans (ACP). At Dudley Group NHS Foundation Trust (DGFT) the GSF has been implemented trust-wide and this allows an opportunity to evaluate the impact that implementation has had on rates of Advance Care Planning for patients who have a GSF code against those who do not. Methods All adult inpatients on 01/09/2023 at DGFT who died in the preceding 12 months in any setting were retrospectively reviewed, to identify if, on 01/09/23, each patient had a recorded GSF Code, date of death (if applicable) and evidence of ACP either in hospital or in the community. Results 216/647 inpatients on 01/09/23 died within the following year in any setting (33.4%). Of the 216 inpatients who died 104 had been GSF identified on 01/09/2024 (16%). Furthermore, 70 patients had documented evidence of ACP in hospital and 159 patients had evidence of ACP in community. When analysing the data further of the 104 patients GSF identified 54% (56/104) had evidence of ACP. For the 112 patients not GSF identified there was no evidence of ACP during their hospital admission. 52 patients had evidence of ACP in both hospital and community settings; 42 (81%) of these patients had a recorded GSF code on 01/09/23. Conclusion This retrospective review has shown that implementation of GSF in hospital increases the rate of recorded ACP in hospital settings to a greater extent than those without a recorded GSF status. Having a GSF status also increases the chance of evidence of ACP in both hospital and community settings.
