65 Supporting wards to achieve gold standards framework accreditation

Abstract: Background At Dudley Group NHS Foundation Trust (DGFT) we have implemented the Gold Standards Framework (GSF) to support identification of patients in the last year of life and development of an individual plan of care. Since implementation commenced in 2019, 11 adult inpatient wards have achieved GSF accreditation with Coronary Care and Critical Care both being the first speciality wards nationally. The Specialist palliative care team (SPC) support all wards in their ambition for GSF accreditation and re-accreditation, and this review demonstrates how this is achieved. Method Using PowerBI, local metrics regarding GSF are reviewed regularly and reported back to individual wards. An action plan is then developed at a ward level with GSF champions and SPC to improve key metrics and outcomes regarding end-of-life care. Once a wards metrics demonstrate the required improvements, this allows SPC to develop a plan for the ward to apply for GSF accreditation. Results Examples include working with Orthopaedics, where a local ward plan to improve identification and coding of GSF increased from 20% to 50% over several months, with the next emphasis on improving rates of advance care planning. For the acute admission ward (which has >1200 admissions per month), the plan was to improve identification rates first, this has increased from 5% to 18% over the last 2 years. Conclusion The ambition at DGFT is that a minimum of 1 new ward achieves GSF accreditation annually and all wards achieve re-accreditation every 3 years. By analysing local ward metrics and developing individual action plans, SPC can support all ward areas to improve their own metrics and outcomes. This allows SPC to support a rolling programme of ward ready to apply for national GSF accreditation.
