63 Communicating gold standard framework with primary care and community teams

Year: 2025
Type: conference-paper
Abstract: Introduction A third of hospital patients are in their last year of life and almost 50% of people die in hospital. At Dudley Group NHS Foundation Trust (DGFT) we have implemented the Gold Standards Framework (GSF) Trust wide. A new discharge template has been developed using the ‘GREAT’ acronym (G – GSF register, R – resuscitation status, E – end of life care medications, A – advance care planning and T – treatment escalation plan) to support communication back to community teams following a hospital admission. Method Random sample of 40 discharge summaries for patients identified as GSF during admission during March and June 2024 were identified. The summary of admission was reviewed noting which elements of the GREAT acronym were being used either in the template or in other places within the document. Results Majority of patients had a non-cancer diagnosis (70%) and the remaining 30% cancer diagnosis. The GSF coding was documented on the summary of admission in 93% of cases (37/40). Ninety eight percent of patients identified as GSF during admission had a DNACPR form on the summary of admission. Furthermore, 100% (11/11) who required anticipatory medication to support discharge had these in place. Discussion It is essential that discussions held in hospital regarding end of life care alongside identification and coding of GSF is communicated effectively with Primary Care ensuring clear co-ordination of care across settings. Introduction of the GREAT acronym has facilitated in communicating vital aspects of an individuals end of life care on their summary of admission and identified areas for education including advance care planning and treatment escalation.
