Abstract: Anti-synthetase syndrome is an autoimmune connective tissue disease described as a myositis characterized by the presence of an autoantibody directed against aminoacyl-tRNA synthetases. Although this disease is generally associated with pulmonary and muscular manifestations, renal involvement is less frequent and less documented. We present the case of a young patient in whom the combination of glomerular nephropathy and pulmonary involvement revealed an anti-synthetase syndrome. A 33-year-old man with no particular medical history presented with a glomerular syndrome, consisting of albuminuria of 1.2g/24h and microscopic hematuria, associated with pulmonary involvement, represented by diffuse infiltrative pneumopathy, and muscular involvement. The immunological workup was positive for anti-Jo1 antibodies, and the diagnosis of anti-synthetase syndrome was made. The patient received immunosuppressive treatment, leading to a favorable disease progression. Early diagnosis and appropriate immunosuppressive treatment can improve the prognosis of these patients.
