Bulky Left Ventricular Masses in a Young Healthy Female

Year: 2025
Type: article
Study design: Case Report
Abstract: Background: Bulky, symptomatic left ventricular (LV) masses represent an uncommon clinical entity without robust, high-quality data to guide management.Case: A 27-year-old female with no medical history presented after a syncopal episode while 8 months pregnant.Echocardiogram was notable for a 5.3 cm LV mass.Cardiac magnetic resonance imaging (CMR) demonstrated two large multilobulated masses involving the inferior wall of the LV.She had dyspnea with minimal exertion, night sweats, palpitations, and a 23-pound weight loss.Further workup was notable for positron emission tomography (PET) scan with only mild peripheral fluorodeoxyglucose (FDG) uptake surrounding the LV masses without definable metastatic process and coronary computed tomography (CT) angiography with CAD-RADS 0.Decision-making: We considered biopsy given concern about mass resection with good margins; however, because there was no evidence of metastatic disease and the patient was having severe symptoms due to mass effect of the LV lesions the patient underwent LV muscle en block dissection with mass removal. Conclusion:Final pathology was consistent with two cardiac fibromas.Multimodality imaging plays a crucial role in evaluation of intracardiac masses and guiding management decisions.
