Abstract: Background: In advanced melanoma, treatment options remain very limited in patients with leptomeningeal disease and brain metastases. Here, we report a case of a 40-year-old female patient with BRAF V600E-mutated, stage IV cutaneous melanoma demonstrating rapid and significant clinical and radiological benefit under a novel therapeutic strategy combining pembrolizumab (PD-1 inhibition), olaparib (PARP inhibitor), and intrathecal nivolumab.
