Abstract: VEXAS syndrome is a newly described autoinflammatory and haematologic condition that has variable cutaneous and systemic presentations. We present a case of VEXAS syndrome in a 63-year-old male with treatment refractory pyoderma gangrenosum and a complex dermatologic and systemic history. We hope it informs and encourages dermatologists to consider early diagnostic testing for VEXAS syndrome in any male over 50 years of age with a neutrophilic dermatosis and unexplained autoinflammation.
