Objective: Intrahepatic cholangiocarcinoma (ICC) represents a heterogeneous group of tumors with an extremely poor prognosis, primarily originating from the epithelial cells of the bile duct. Owing to its subtle symptoms, ICC is frequently diagnosed at an advanced stage, resulting in an unfavorable prognosis. Early diagnosis, prognosis prediction, and effective treatment remain imperative to enhance the quality of life for ICC patients. Exosomes, a subtype of extracellular vesicles, are small phospholipid bilayer vesicles capable of transporting a variety of biomolecules, including proteins, RNA, DNA, metabolites, and lipids. This endows them with the potential to serve as cancer biomarkers and regulators participating in diverse biological activities within the human body. This article comprehensively reviews the role of exosomes in the mechanisms underlying the occurrence and development of intrahepatic cholangiocarcinoma, highlighting their prospective applications as disease biomarkers.
