Pathological Proximal Femur Fracture in a Patient with Distal Renal Tubular Acidosis: A Case Report

Year: 2025
Type: article
Study design: Case Report
Abstract: The incidence of pathological fractures is on the rise, resulting in significant morbidity for patients and their families, while also imposing a considerable burden on the healthcare system. Early identification of at-risk patients is crucial for reducing costs and enhancing quality of life. Although various renal pathologies have been associated with the risk of pathological fractures, complications linked to renal tubular acidosis (RTA) are seldom reported. However, prompt action and adequate patient education can significantly mitigate the associated morbidity. Chronic metabolic acidosis in distal renal tubular acidosis has been shown to contribute to increased bone resorption and osteopenia, ultimately leading to a loss of bone mineral content. This case report discusses a patient who suffered a pathological proximal femur fracture due to undiagnosed renal tubular acidosis. The patient presented with hypokalemia, osteomalacia, and metabolic acidosis.
