Abstract: Background. Chronic recurrent aphthous stomatitis (CRAS) is one of the most common lesions of the oral mucosa, occurring in 25 % of the population. The frequency of relapses is quite high and reaches 50 %. Treatment of CRAS is mainly limited to the administration of only local anti-inflammatory drugs, which does not allow achieving stable remission of the disease and avoiding frequent relapses. Research objective: improving the effectiveness of the treatment for chronic recurrent aphthous stomatitis and increasing the duration of remission. Materials and methods. A retrospective analysis of the diagnosis and treatment of CRAS was conducted in 92 patients aged 18 to 65 years (mean age 32.4 ± 1.8) from 2019 to 2023. Depending on the diagnostic and treatment tactics, the patients were divided into two groups. Patients in the main group, after an objective examination, underwent a biopsy of the aphthae with morphological examination. According to the results of the pathomorphological study, they were prescribed specific therapy in addition to local treatment of СRAS. In the comparison group, an objective assessment of aphthae was performed without biopsy, and the therapy was only local. Results. According to the results of the morphological study of aphthae in the main group, granulomatous inflammation was detected characteristic of inflammatory bowel diseases. The average duration of remission in the main group was 11.35 ± 0.18 months due to the administration of both local anti-inflammatory treatment and mesalazine preparations, while in patients in the comparison group who received only local treatment, it was 6.45 ± 0.65 months. Conclusions. The presence of СRAS with detected granulomatous inflammation of the oral mucosa is characteristic of chronic inflammatory bowel diseases, which justifies the prescription of both local and general pathogenetic treatment, which reduces the frequency of relapses to 11.5 % in the main group versus 51 % in the comparison group.
