Abstract: Introduction: "Infertility is defined as the failure to conceive within one or more years of regular unprotected coitus." Many factors are responsible for causing infertility one among them is thin endometrium. Thin endometrium is a significant factor contributing to female infertility, which affects approximately 10-15% of couples worldwide. Ayurvedic interventions such as Phalaghrita Uttarbasti (intrauterine application) and oral administration (Pana) have been traditionally employed to enhance endometrial health and improve fertility. This study aimed to evaluate the individual and combined efficacy of these methods in patients with infertility associated with thin endometrium. Methods: A randomized controlled trial was conducted with 20 female patients diagnosed with thin endometrium (< 7 mm). Participants were divided into two groups:Group A: 10 patients received Phalaghrita Uttarbasti (3ml on days 6, 8, and 10 of the menstrual cycle for 3 cycles).Group B: 10 patients received a combination of Phalaghrita Uttarbasti along with oral administration (10 ml twice daily with warm milk for 3 cycles).Endometrial thickness was measured using transvaginal ultrasound before and after treatment. Secondary outcomes, including conception rates and menstrual regularity, were observed over six months. Results: Group A (Uttarbasti) showed a mean increase in endometrial thickness from 5.3 mm to 9.0 mm, with a conception rate of 50%. Group B (Uttarbasti along with Pana) demonstrated a greater improvement, with mean thickness increasing from 5.4 mm to 9.5 mm and a conception rate of 65%. Both groups experienced improved menstrual regularity, with Group B showing more pronounced effects.Discussion: The combination of Phalaghrita Uttarbasti along with oral administration (Group B) was more effective than Phalaghrita Uttarbasti (Group A) alone in enhancing endometrial thickness and improving fertility outcomes. The synergistic effect of the dual approach likely provides both localized and systemic benefits, addressing multiple factors contributing to infertility. Conclusion: Phalaghrita administered via Uttarbasti along with oral administration together demonstrates superior efficacy in managing infertility related to thin endometrium.
